The developmental and aging mechanisms of the vestibular system
Cochlear implantation is a safe surgical technique associated with a low rate of complications. Among these, postoperative vertigo is one of the most observed. Despite its high incidence in the general population, Benign Paroxysmal Positional Vertigo (BPPV) after cochlear implantation is rarely described.
Generally, treatment by repositioning maneuvers is sufficient; however, some patients may experience recurrent and/or persistent paroxysmal vertigo.
In this Instructional Course we describe the role of Intratympanic gentamicin in the treatment of persistent BPPV into the implanted ear.

High-frequency bone-conducted vibration is not merely “very fast motion”: for a sinusoid of fixed displacement, jerk scales with ω³ and becomes the dominant mechanical feature at ~60–100 Hz. This keynote will link the physics of vibration/jerk to the dual-channel vestibular periphery, highlighting the preferential activation of Type I hair cells, calyx synapses and irregular afferents. Clinical translation will focus on how BCV-VEMPs and skull vibration-induced nystagmus (SVIN) probe this transient pathway in unilateral vestibular loss, Ménière’s disease and third-window syndromes.
