This presentation reviews the principles of totally endoscopic ossiculoplasty, focusing on sound-transfer mechanics, prosthesis biomechanics, and practical decision-making in type III and IV tympanoplasty. The transcanal endoscopic approach provides superior visualization of the oval window, retrotympanum, and prosthesis alignment while preserving middle-ear ventilation pathways. Key concepts include ossicular and acoustic coupling, the functional importance of round window preservation, and biomechanical factors influencing long-term outcomes, including prosthesis material, alignment, length, and tension. Reconstruction strategies are discussed according to ossicular chain status, highlighting a practical framework for selecting titanium prostheses and achieving stable, durable hearing restoration.
