Tympanoplasty: Techniques, approaches, and outcomes

<p>This presentation reviews the principles of totally endoscopic ossiculoplasty, focusing on sound-transfer mechanics, prosthesis biomechanics, and practical decision-making in Type III and IV tympanoplasty. The transcanal endoscopic approach provides superior visualization of the oval window, retrotympanum, and prosthesis alignment while preserving middle-ear ventilation pathways. Key concepts include ossicular and acoustic coupling, the functional importance of round window preservation, and biomechanical factors influencing long-term outcomes, including prosthesis material, alignment, length, and tension. Reconstruction strategies are discussed according to ossicular chain status, highlighting a practical framework for selecting titanium prostheses and achieving stable, durable hearing restoration.</p>

<p>Dr. Gherasie is an otologic surgeon, academic faculty member, and pioneer of exclusive endoscopic ear surgery in Romania.</p> <p>She practices at the “Prof. Dr. D. Hociota” Institute of Phonoaudiology and Functional ENT Surgery and is affiliated with Carol Davila University of Medicine and Pharmacy, Bucharest. Her work focuses on performing complex procedures, including advanced tympanoplasty, cholesteatoma surgery, stapedotomy, and middle ear vascular tumors, expanding minimally invasive otologic surgery while integrating patient-reported outcome measures into surgical evaluation.</p>
