In this course, in which i aim to present surgical techniques for blepharoptosis, i will first outline a general clinical approach to patients with upper eyelid ptosis, provide a detailed preoperative evaluation, and explain which surgical treatment i choose for which case. In the final section, i will share my high-quality operation videos demonstrating müller muscle–conjunctival resection, levator muscle procedures, and frontalis suspension with a silicone sling.

Evidence-based surgical approaches in lower eyelid blepharoplasty

Facial rejuvenation and beautification in all breeds is a true challenge for an artistic surgeon, where the surgeon's art and expertise play a fundamental role in making accurate diagnoses and deciding the appropriate technique and type of surgery for each patient. This is done in such a way that we can rejuvenate and beautify the face in a harmonious way across all three facial thirds, achieving natural results, leaving patients looking younger, more beautiful, with an attractive, relaxed look, and not looking as if they've had surgery. Which is a multiethnic city where whites, afro-descendants, indians and different types of mixed races live due to the crossing between the different races and to beautify and rejuvenate faces, the artist surgeon has to perform one or more simultaneous facial plastic surgeries on his patients, such as: forehead surgery, blepharoplasty, direct eyebrow surgery, rhinoplasty augmentation or reduction as the case may be, mentoplasty augmentation or reduction, double chin liposuction with or without cervicoplasty as the case may be, cheiloplasty, otoplasty, gingival smile correction, bichectomy and cervicofacial rhytidoplasty.

Subciliary lower eyelid blepharoplasty has evolved to achieve a more harmonious, natural appearance with a fuller, unoperated look, while minimizing complications. Whereas lower eyelid malposition was once common, its incidence has been markedly reduced through careful preoperative evaluation, meticulous surgical planning, precise technique, and attentive postoperative care. Prophylactic maneuvers that maintain or reinforce lower lid support include preservation of the pretarsal orbicularis oculi muscle, conservative skin and muscle resection, and suspension of the orbicularis oculi muscle and/or tarsus to the periosteum of the lateral orbital rim. Releasing the orbicularis retaining ligament and transposing orbital fat over the rim, rather than excising it, smooths the lid–cheek junction, corrects the tear trough deformity, and reduces the appearance of lower eyelid fat prolapse. This presentation outlines a comprehensive approach to achieving a smooth lid–cheek transition while preserving or restoring lower lid support and provides detailed postoperative guidelines to optimize recovery.
