Rhinoplasty is considered "The Queen" of Plastic Surgeries, since the Artistic Surgeon has to correct different types of noses that depend on the age, sex and race of the patient; as well as if the patient has a history of previous surgeries and direct trauma to the nose.
In order for a Rhinoplasty to be successful, the Surgeon has to diagnose the previous anatomical details of the nose to be operated, verify the state of the nasal Septum, nasal turbinates, as well as the presence of nasal polyps, Rhinosinusitis and the history of previous nasal surgeries; to successfully perform a functional and aesthetic Rhinoplasty simultaneously.
To obtain successful and predictable results in a Rhinoplasty, it is essential that the Facial Plastic Surgeon and Otolaryngologist, has at least 10 or more years of experience practicing this surgery in different types of noses and knowing multiple surgical techniques to correct the different nasal deformities that are found. The planning and decision of the surgical technique is based on the precise diagnosis of the functional and aesthetic deformities of the nose, that is the art of the Facial Plastic Surgeon, to achieve beautiful and natural results in a Multi-Ethnic Rhinoplasty which is a real challenge for the best Facial Plastic Surgeon.
The author shares his experience of 36 years practicing multi-ethnic Rhinoplasties in the city of Cartagena de Indias - Colombia.
Objective:
To demonstrate that the result of a multiethnic Rhinoplasty depends on the skill of the Facial Plastic Surgeon in accurately diagnosing the deformities of the nose of his patients to decide according to his expertise and experience operating multiethnic noses which is the appropriate technique for each particular case and achieve harmonious and natural aesthetic and functional results suitable for each type of multiethnic nose operated; which many times must be associated to the practice of other facial plastic surgeries that the patient needs (Mentoplasty, Otoplasty, Bichectomy, Cervicoplasty, etc.)

Septal extension graft is a workhorse for tip rhinoplasty. It has several functions and may be used with different purposes. Furthermore, the shape and spacial position of the graft may be different and should be chosen according to the specific role of the septal extension graft in the specific clinical case being dealt with.
Maxillary sinus is the biggest sinus of paranasal cavity. In the endoscopic era it is a little bit neglected. Endoscopic approaches to maxillary sinus can be challanging, so a rhinologist must master different techniques and approaches to different pathologies.
