Complex revision rhinoplasty and cleft nose correction require advanced reconstructive strategies due to severe scarring, tissue loss, and functional compromise. This presentation focuses on the use of flap techniques combined with dermal and composite grafts to restore lining, support, and external contour. Surgical videos will demonstrate practical applications and pearls from challenging cases.
Outcome Objectives
Participants will learn to:
1. Identify indications for dermal and composite grafts in revision and cleft nose surgery.
2. Understand how local flaps can be combined with grafting for improved function and stability.
3. Apply these principles to achieve natural and long‑lasting results in complex nasal reconstructions.
Background
Traditional techniques are often insufficient in secondary and cleft rhinoplasty. Dermal and composite grafts, together with local flap rearrangements, provide reliable tools to correct asymmetry, improve airway function, and enhance aesthetics. This session will share practical experience to expand the surgeon’s armamentarium in the most demanding nasal cases.
This study emphasizes the critical role of the septal extension graft in achieving stable tip projection and definition in asian rhinoplasty. Due to the weaker cartilage framework and thicker skin envelope in asian noses, SEG provides essential structural support and long-term shape control. The research aims to assess its effectiveness in maintaining aesthetic balance and functional stability postoperatively.
Facial nerve paralysis has significant effects on a patient’s daily social interaction, nevertheless, the functional deficits that the patient experiences with oral incompetence, speech and deglutition can be debilitating. We describe a novel technique “valentine anguloplasty” which involves the excision of skin in the shape of a valentine at the corner of the upper lip on the paralytic side. The technique has been illustrated in the results section. We found that valentine anguloplasty has many advantages, including its ability to lift the corner lip improving static symmetry as well as oral competence. It also provides access to tunnel fascia lata sling from mid-face to anchor to the upper lip, lower lip and oral commissure to help provide further lift to the droopy corner of the lip on the paralytic side.
The nose framework is an important structure that carries the nose, gives the nose its shape and provides breathing properly to pass through it. It is important for nasal surgeries to be performed it with appropriate strength and regularity. One of the most frequent and important reasons for revisions of nasal surgeries is an insufficiently strong and unbalanced nose framework. The reason for important pathologies such as nostril asymmetries, pinching deformity, polly beak deformity, saddle nose deformity, and nasal valve collapse that we frequently see in revision cases is that the nose framework does not have sufficient strength and regularity. In almost all cases opened for revision, there is a need to correct the nose skeleton. Therefore, in revision surgeries, the most important stage of achieving aesthetic and functional progress is to reconstruct the nose framework. In this presentation, we would like to convey our approach and experience on this subject to the participants with both surgery videos and animation videos of our techniques, some of which have been published in international journals.
