Recurrence after primary treatment of head and neck cancer is associated with significant morbidity and mortality. In terms of treatment options, surgery is considered in many cases to offer the best or only chance of a curative outcome. However, salvage surgery in the Head and Neck is complicated and often associated with poor outcomes and survival. Furthermore, there is a lack of good evidence basis due to heterogeneity in series and variability in management. The aim of this lecture is to review the current status of salvage surgery for recurrent Head and Neck cancer, to review the evidence regarding optimal patient selection and prognostication, and to explore novel alternative approaches for cases of borderline resectability.
