Gülin Gökçen Kesici

<p>Dr. Gülin Gökçen Kesici graduated from Hacettepe University Faculty of Medicine in 2005 and completed her residency in Otorhinolaryngology at Ankara University in 2012. She holds both a Master’s and Ph.D. degree in Audiology, completed in 2022. In 2020, she received the title of Associate Professor. Her clinical and academic interests include otology, neurotology, pediatric otolaryngology, and vestibular and auditory diagnostics. She currently works as a faculty member and department head at Ankara Medipol University.</p>

Vertigo is often a challenge for the generalist ENT practitioner. Even with computer tools, doubt can persist between central and peripheral origin and about the need to carry out a maneuver for VPPB. The HINTS protocol, applied in the emergency rooms, has been the reference for several years and is more sensitive than an MRI within the first 48 hours. Many vertiginous patients are mishandled, making them worse. The HINTS protocol should be applied as well in general ENT. After a quick introduction on nystagmus, will follow a step by step videos presentation of the HINTS protocol.

Description, Outcome Objectives and Background:

Positional Vestibular Migraine (PVM) is an under-recognized variant of vestibular migraine in which symptoms are provoked primarily by head position changes, often mimicking benign paroxysmal positional vertigo (BPPV). Clinicians may misattribute these episodes to canalolithiasis, leading to ineffective repositioning maneuvers and delayed migraine treatment.This lecture will present diagnostic criteria, video illustrations, and recent insights into pathophysiology and neuroimaging in PVM. It will also discuss differential diagnosis from central and peripheral causes of positional vertigo, emphasizing when to suspect PVM in challenging cases.

- To define clinical features distinguishing PVM from typical BPPV
- To illustrate diagnostic challenges and potential overlaps
