Background
The maxillary sinus is a key anatomical structure in endoscopic sinonasal surgery and presents a wide spectrum of surgical indications, including inflammatory, odontogenic, tumoral, and traumatic diseases. Over recent decades, surgical management has evolved from conventional middle meatal antrostomy to extended and function-preserving endoscopic approaches that allow comprehensive access to all sinus compartments. Advances in anatomical understanding, imaging, and endoscopic techniques have led to the development of approaches such as endonasal maxilectomy or as the prelacrimal route, which offers broad exposure of the maxillary sinus while preserving the inferior turbinate and nasolacrimal duct. Despite these advances, appropriate selection of the surgical approach remains challenging and highly dependent on surgeon expertise.
Description
This instructional course is designed as a structured and practical overview of contemporary endoscopic surgical techniques for the maxillary sinus. The course will systematically review the spectrum of approaches, including standard and extended middle meatal antrostomy, transnasal endoscopic medial maxillectomy, and the prelacrimal approach. Emphasis will be placed on surgical anatomy, indications, technical nuances, limitations, and potential complications of each technique. Comparative analysis will be used to highlight the advantages and drawbacks of different approaches, with particular attention to functional preservation and surgical efficacy. The course will incorporate anatomical illustrations, surgical videos, and case-based discussions to facilitate clinical decision-making.

At the end of this course, participants will be able to:
Identify key anatomical landmarks and variations relevant to maxillary sinus surgery.
Understand the indications and contraindications for each endoscopic approach to the maxillary sinus.
Select the most appropriate surgical technique based on pathology, extent of disease, and surgical goals.
Appreciate the role of the prelacrimal approach in comparison with traditional and extended techniques.
Apply a rational, stepwise strategy for maxillary sinus surgery aimed at maximizing outcomes while minimizing morbidity.
