Grafts In Tympanoplasty

  • Objective: To evaluate the anatomical and functional outcomes of a novel grafting modification: endoscopic annular chondroperichondrial tympanoplasty.

  • Methods: This retrospective study evaluated 40 chronic otitis media patients operated on between 2015 and 2022. Tragal cartilage was harvested, its center was circularly excised to leave a 2–3 mm ring, and a 1 mm peripheral perichondrial edge was engineered along with a malleus slot. Grafts were placed via a transcanal endoscopic approach using the under-over technique.

  • Results: The overall graft integration rate was 95% (38/40 intact grafts) at the 6-month follow-up. The mean preoperative PTA-ABG significantly improved from 22.1 dB to 5.7 dB postoperatively (p < 0.05), yielding a mean closure of 16.4 dB. No major complications were noted.

  • Conclusion: This novel modification bridges the gap between acoustic excellence and structural stability, offering high anatomical success and optimal hearing reconstruction in modern otology.

Prof. Dr. Semsettin Okuyucu is an ENT Professor at Medicana International Ankara Hospital with a strong clinical focus on Transcanal Endoscopic Ear Surgery (TEES). A graduate of Cerrahpasa Faculty of Medicine (English Program), his academic career includes serving as a senior faculty member at Hatay Mustafa Kemal University (2004–2023). He has completed advanced fellowships at Johns Hopkins University, University of Pittsburgh, University of Florida, and the University of Rome. As an active member of the Turkish Society of Otology and Neurotology and the Endoscopic Ear Surgery Working Group, his work is dedicated to utilizing the limits of TEES. An invited speaker at premier global symposia, his clinical TEES presentations and customized graft innovations have been featured at the prestigious Politzer Society meetings and APSCI, contributing to the advancement of minimally invasive hearing reconstruction.
