 "><strong>Background &amp\; Objectives: </strong>Vasomotor rhinitis (VMR)
is a non-allergic chronic rhinitis characterized by vascular dysfunction w
ithin the nasal cavity and autonomic nervous system imbalance. In cases of
medically refractory and resistant VMR\, Endoscopic Transnasal Vidian Neu
rectomy—an advanced autonomic denervation surgery—is performed to cont
rol hypersecretion and vasodilation. This video presentation aims to demon
strate the vidian neurectomy surgical procedure guided by <strong>anatomic
al landmarks\, and to</strong> <strong>discuss the deep anatomical structu
res of the pterygopalatine fossa\, vidian nerve\, and paranasal region</st
 rong> from a surgical perspective.</p><p style="text-align: justify\; marg
 in-left: 0px!important\;"><strong>Surgical Technique &amp\; Methods: </str
ong>The procedure is performed endoscopically under general anesthesia. Fo
llowing the identification of the lateral nasal wall landmarks\, a mucoper
iosteal flap is elevated to access the sphenopalatine foramen. Precise dis
section is required to expose the medial boundary of the pterygopalatine f
ossa. Upon localization of the vidian canal (<em>canalis pterygoideus</em>
)\, the vidian nerve—comprising fibers from the <em>nervus petrosus majo
 r</em> and <em>nervus petrosus profundus</em>—is identified. Selective n
eurectomy is performed at this level\, followed by measures to ensure the
stability of the surgical site and prevent neural regeneration. The proced
ure concludes with the repositioning of the mucosal flap and appropriate s
 tabilization.</p><p style="text-align: justify\; margin-left: 0px!importan
 t\;"><strong>Conclusion &amp\; Significance: </strong>This video presentat
ion provides a comprehensive\, step-by-step demonstration of endoscopic tr
ansnasal vidian neurectomy. By highlighting the critical anatomical landma
rks of the pterygopalatine fossa\, this work aims to contribute to the sur
gical success and safety of autonomic denervation procedures. It serves as
a practical anatomical guide for clinicians managing refractory vasomotor
  rhinitis.</p><p style="text-align: justify\; margin-left: 0px!important\;
