Management of Resection Margins in Oral Cavity Squamous Cell Carcinoma

The management of surgical margins in oral cavity squamous cell carcinoma is a subject of ongoing debate in current literature. While most clinical guidelines define resection margins based on geometric parameters—typically >5 mm in planar measurement or >7 mm³ in three-dimensional assessment—this approach may not account for the complex anatomical and biological variability of the oral cavity. Modern approaches advocate for a more nuanced view that considers the variability in tumor subsites and structural anatomy, as well as key factors such as the tumor infiltration front, the mode of tumor progression (mucosal versus stromal), and the involvement of critical anatomical spaces like the sublingual areas, the T-N tract, and the infratemporal fossa.

There is also ongoing discussion regarding the most effective methods for evaluating surgical margins, including imaging techniques, bioendoscopy, and intraoperative frozen section analysis, with no standardized decision-making algorithm currently established.


The purpose of this proposal is to present, thtrough the discussion of case reports, recent evidence on the management of resection margins in oral cavity squamous cell carcinoma through selected clinical cases. These illustrate various patterns of tumor spread, including superficial progression, deep infiltration, invasion of the masticatory space, and involvement of bone or skin. The goal is to highlight the importance of understanding tumor biology and recognizing anatomical pathways of least resistance. These elements are essential for improving surgical planning and outcomes, and for developing more tailored and effective treatment strategies.

Erdoğan Özgür, MD, Associate Professor of Otorhinolaryngology, is a faculty member in the Department of Otorhinolaryngology at Dokuz Eylül University Faculty of Medicine, İzmir, Türkiye. His clinical practice and academic focus are primarily dedicated to head and neck oncology.

Dr. Özgür completed his PhD in the field of head and neck cancer research, with particular interest in tumor biology and immune checkpoint pathways in laryngeal carcinoma. His research focuses on surgical oncology, prognostic factors, and translational aspects of head and neck malignancies.

He is actively involved in clinical research and surgical management of head and neck cancers and regularly participates in national and international scientific meetings.
