Skull vibration induced afternystagmus - a new clinical indicator of superior canal dehiscence
The after-nystagmus (AN) is a distinctive sign in superior canal dehiscence (SCD) and is not observed in total unilateral vestibular lesion (TUVL) patients but only in some Third Mobile Window Syndrome (TMWS). It likely relies on a cupula deflection secondary to endolymph flow induced by the vibration and stimulation of regular discharging neurons by the bone-conducted vibrations (BCV) in SCD but not TUVL patients. It is a simple, specific, robust sign of SCD and TMWS, and its sensitivity is comparable to air-conducted Tullio phenomenon and other compressional tests. AN interest is increased when associated with observation of other per-stimulatory Nystagmus (PN) characteristics suggestive of SCD. Its observation relies on the contribution of the velocity system integrator.
